Pre-registration is required for class

Upon electronic receipt of the completed on line registration below, a space in class will be temporarily held for your team(s). You will be sent an email that we have your electronic registration on file and to please send your registration fee to the P.O. address listed. Once we receive the registration fee by regular mail, you will receive email confirmation for your class date along with directions to the facility and other information. You are not confirmed until you receive email notification we have received your registration fee. We look forward to seeing you at class. Space is limited and fills quickly so we advise to register early. Fill out completely (* indicates REQUIRED FILEDS), read the agreement below, initial and SUBMIT at the bottom of this form.

With your initials below you agree to the following: I indemnify Diane Rich, Diane Rich Dog Training LLC, Cascade Vista, Cascade Plaza, any instructor, or assistant in class, and any of their contractors or volunteers and hold them harmless from and against any and all claims, losses, liabilities and damage to persons or property, governmental, charges or fines and attorney's fees arising out of the acts or omissions of this workshop/class including but not limited to: interactions with instructors, attendees, volunteers, and/or animals screening or demonstrations involving my companion animal(s) or transportation of myself and companion animal(s) to or from the workshop or within the same site. Refund Policy: Sorry, due to limited class size no refunds for cancellations made 14 or less days of class, no shows, or if asked to leave the class. NO free makeups. No exchanges for another class date. $25 processing/cancellation fee for cancellations before the 14 day cutoff. No refunds, makeups or exchanges for second dog in family or second family member if cancelled at any time or no show. We reserve the right to reschedule class dates due to facility conflicts, illness or group size. You will be offered an alternative date
